Please do the following in

preparation for your blood donation:

• Eat a good meal prior to your donation.

• Drink plenty of water the day before and the day of your donation.

ID is required to be permitted to donate blood. One of three  following methods is acceptable:

1. LifeSource Donor Card                      2. Drivers License/Learners permit/state ID;

3. Donors presenting with only a picture ID may be

accepted if: A through C below has been fulfilled.

A. The donor’s picture is on the piece of ID;

B. The ID is printed with either donor’s first and

last name or first initial and full last name;

C. The donor can verbalize his/her date of

birth and either the last four digits of his/her

social security number or another

government issued ID number.

The above identification options are listed according

to preference; Option “1” being first preference,

Option “2” is second preference. Option “3” is the

last preference.
